BMJ Case Rep Published online: [please include Day Month Year] doi:10.1136/bcr-2014204787 DESCRIPTION The human papillomavirus (HPV) can infect the skin and mucous membranes. Common warts, filiform warts, epidermodysplasia verruciformis, mosaic warts and plantar warts represent clinical manifestations of infection with HPV. Giant warts in the hands are very rare in immunocompetent adults and sometimes by their extension or atypical location represent a diagnostic and therapeutic challenge. The giant tumours caused by HPV are more common in the anogenital region where they are known as Buschke-Loewenstein tumours. No risk factor has yet been identified for giant warts in immunocompetent patients. The authors report this rare case of a giant wart with a linear configuration (figure 1) in an immunocompetent 40-year-old adult in whom clinical presentation resembled a verrucous epidermal naevus. Other differential diagnoses such as cutaneous adnexal neoplasms and squamous cell carcinoma have to be thought. HPV lesions of long duration and subjected to various treatments tend to lose their histology (figure 2) and most of the times HPV DNA cannot be detected by PCR in the histological specimen (as was the case). This fact, associated with the nodular and hyperkeratotic nature of these lesions may explain the poor response to various therapies performed before and a not so marked response to imiquimod. The patient was initially treated with imiquimod during 12 weeks to reduce the size of the lesions and subsequently serial excision of the resistant nodes was carried out.

Molecular Cloning and Characterisation of a Novel Type of Human Papillomavirus 160 Isolated from a Flat Wart of an Immunocompetent Patient

More than 150 types of Human papillomaviruses (HPVs) have been isolated from numerous cutaneous and/or mucosal lesions. Flat wart samples on the face from 36 immunocompetent patients were collected and screened for HPV. From one sample, we cloned a putative novel genotype. The novel type consisted of 7779 bp in length with a GC content of 47.1%, containing open reading frames for putative early...

Plantar molluscum contagiosum: a case report of molluscum contagiosum occurring on the sole of the foot and a review of the world literature.

Molluscum contagiosum (MC) is a skin infection caused by the double-stranded DNA virus of the family Poxviridae that typically presents as flesh-colored asymptomatic umbilicated papules. Plantar MC is uncommon. We describe a 23-year-old man who presented with multiple plantar MC. We also summarize the epidemiologic features of the 34 previously reported patients with plantar MC and discuss the ...

Complete Healing of a Giant Wart in a Severely Immune-Compromised Patient with HIV Infection Treated with Acupuncture

Giant warts are infrequent dermatological viral infections caused by Papillomavirus (HPV) in immune-compromised patients. Treatment may often be difficult and unsatisfactory, either by surgery or cytotoxic agents, because of poor immune control of viral activity in such hosts. Here we report on the case of a patient with advanced and persistent immune suppression caused by HIV disease, who deve...

An unusual, giant and benign condylomaacuminatum lesion on the genital area for more than a decade

Condyloma acuminatum, is a rare sexually transmitted disease. The virus responsible for condyloma is human papillomavirus. HPV-6 and HPV-11 are the most commonly detected HPV genotypes, but at least 20 other HPV genotypes have occasionally been found in genital wart tissue specimens. In fact genital HPV infection is common among sexually active populations. This disease may occur at any age af...
